Version 2026-11-09-draft

Draft. This text has not yet been reviewed by a lawyer and is not final. Items in [square brackets] are still to be filled in.

1. What CRNA Platform is

[Company legal name] (“CRNA Platform”, “we”, “us”) runs a website where CRNAs (certified registered nurse anesthetists) find anesthesia jobs posted by healthcare facilities and staffing agencies. A CRNA keeps one credential file here and shares it for the jobs they apply to, so a facility can review it without collecting the same documents again.

We prepare, check and route information. We are not a healthcare provider, not an employer of CRNAs, not a staffing agency, not a credentialing verification organization, and not a licensing or accrediting body. The facility's governing body alone decides whether to grant clinical privileges, and the facility or agency alone decides whom to book. Nothing we show means a CRNA is approved, credentialed or privileged by us.

2. Your account

You must be at least 18 and give accurate information. Every account uses two-step verification with an authenticator app. Keep your password and authenticator safe; you are responsible for what happens under your account. Tell us promptly if you think someone else has access.

An account belongs to one person. A facility or agency account is opened for the organization by someone authorized to act for it, and that person confirms they have that authority.

3. CRNAs

CRNA Platform is free for CRNAs. On jobs posted directly by a facility you work as an independent contractor of that facility. On jobs posted by an agency, the agency contracts with and pays you, and the facility grants your privileges. We are not a party to those relationships.

You promise that everything you add to your file is true, current and yours, including licenses, certifications, documents and your answers on application forms. Do not add documents that are not yours or that you are not allowed to share.

Your file is shared with a facility or agency only through a sharing grant: a limited profile (first name and last initial, experience, case types, credential status and general area) before you are booked, and your full file after you are booked. Health records are shared only after booking. A grant ends 30 days after the job, and you can stop sharing earlier from your account. Your access history shows who opened what.

You agree that we may search public registries and exclusion lists using your name and NPI when you save your profile, and again about once a month while you have an account: the NPI registry (NPPES), the OIG exclusion list (LEIE) and SAM.gov exclusions. We may add other public lists. A name match is only ever shown as a “possible match, under review” until a person at CRNA Platform decides whether it is you; we do not call anyone excluded on a name match alone.

To confirm a booking you need your National Provider Identifier (NPI) on your profile and a registry check that matches your name. An NPI belongs to one person and one account. Your record of late cancellations and confirmed no-shows is tied to your NPI and name, and stays with them if you close your account and later open another. There are no buyout fees and no non-compete in any contract or flow on CRNA Platform. We never charge you to be hired.

4. Jobs, booking and rates

Every listing states a guaranteed daily rate and an hourly rate for time past 8 hours a day. You may ask for a different daily rate up to two times per job; the posting organization has 48 hours to accept or keep the listed rate. Offers are private between the two of you. An accepted rate becomes the guaranteed daily rate for that booking.

A booking happens when you apply (or accept an invitation), the facility or agency picks you, and you confirm within 48 hours. Other applicants are then told the job is filled. A facility or agency may pick one applicant and is never obliged to pick anyone.

Messages before booking block phone numbers and email addresses. After booking you may share them.

5. Cancellation protection

Each listing shows the cancellation terms it was published with, and those terms stay with that listing. Unless the listing says otherwise: if the facility or agency cancels a booked job less than 72 hours before the first day, the CRNA is owed one guaranteed day at the listing's rate. The CRNA bills that amount directly; CRNA Platform does not collect or pay it. A CRNA who cancels owes nothing, and the job reopens if it has not started. Cancelling closes the CRNA's file to that organization.

6. Money

CRNA Platform never holds or moves money for shifts. The CRNA invoices the facility or agency directly (the invoice tool on the site pre-fills days at the agreed daily rate plus any overtime, and the CRNA sends it themselves), and the facility or agency pays the CRNA directly. We do not store invoices or bank details.

Facilities and agencies pay us: (a) a fee of 7% of the guaranteed daily rate multiplied by the booked days, never including overtime, charged once for each CRNA and facility pair on their first assignment together; and (b) a flat monthly subscription after two free months, at the price shown when you subscribe. Texas sales tax applies where required. [Attorney: payment terms, refunds, late payment, taxes.]

7. Facilities and agencies

You may use a CRNA's file only to decide on booking and privileges for the job it was shared for, and you must keep it confidential and secure. Do not copy it into other systems or share it with others except those who must see it for credentialing, as the law requires. Stop using it when the grant ends.

You are responsible for your own credentialing decisions, for checking photo identification at onboarding (we match name, license and certification records but do not confirm identity with an ID document), for any background check you order, and for following the laws that apply to you, including employment, anti-discrimination and consumer-reporting laws.

You are responsible for paying the CRNA as agreed. We do not guarantee that a CRNA will be available, qualified or suitable.

8. What our checks mean

We describe a file as “complete and pre-checked” when every item on the facility's checklist is present and our checks have run. That is not a verification, approval or guarantee. “No match found” describes a search of a list on the date shown. Licenses and NBCRNA certification are looked up on the issuing body's public lookup by a person at CRNA Platform and the result is saved with its date; other documents are checked by the facility.

The public lists we search change, and a name match without a date of birth is never proof. You must make your own decision before booking or granting privileges.

9. What not to put on the site

Do not upload or enter Social Security numbers, bank details or any information that identifies a patient. Case logs must be de-identified. If you add something like that by mistake, tell us and we will remove it.

Do not use the site to break the law, harass anyone, misstate your credentials, scrape or copy data, probe or bypass our security, or contact people outside the platform in ways the platform is designed to prevent before a booking.

10. Your content

You keep ownership of what you add. You give us a limited licence to store it, process it and show it to the people you share it with, only to run the service for you. We do not sell your data.

11. Availability and responsibility

The service is provided as it is, and we work to keep it available and accurate but do not promise it will be uninterrupted or error-free, or that you will find work or fill a position. Nothing here is legal, medical or credentialing advice.

[Attorney: limitation of liability, indemnity, and dispute resolution (arbitration or courts, venue, jury waiver, class waiver).] These terms are governed by the laws of the State of Texas [attorney to confirm].

12. Ending your account and changing these terms

You may stop using CRNA Platform at any time and ask us to close your account at [legal contact email]. We may suspend or close an account that breaks these terms or puts others at risk. Some records, such as the audit log of who opened a file, are kept as described in the Privacy Policy.

We may change these terms. When we do, we will ask you to accept the new version before you keep using the site, and we record which version you accepted and when.
